1963 Danelectro guitar

I am looking to be reunited with a black and white Danelectro that I owned once in 1977 and again in 1993. It was ultimately sold through a used instrument shop in Portland Me in 1993 or 1994. It has the wide headstock, not the coke bottle style. When I sold it, it was in a rectangular hsc. There are a couple other ways I can identify it. Attached is a pic of the actual guitar, taken in 1993.
