Teleflora Crystal Taper Candle Holder

I am looking for a Teleflora crystal taper candle holder. It has 3 pieces to it. The crystal base, the crystal hurricane piece and a gray hard plastic liner that fits over the crystal base. The liner is used to put floral foam so you can make a floral arrangement. I believe it is from the early 1990's.
